I was sent a sleeve of Titleist test golf balls a while back. I live in New Jersey and received the balls 2 days before hurricane sandy hit my area hard.  I was finally able to go and hit them is there any way I can still do the survey for Titleist?  I lost the sheet it came with and don't know if there still excepting responses I hope they are I would really like to participate thanks.
